The interesting comments by Fusagawa et al focus our attention on 2 relevant issues regarding the management of the umbilical venous catheter (UVC). First, there is a difference between the migration of the tip of an UVC and the dislodgement of its external tract. These 2 phenomena may occur independently or simultaneously, but they have different causes. Tip migration may occur even without any apparent dislodgment of the external tract of the UVC, and migration may be caused by variations in abdominal pressure, lung volume, or level of sedation; periodic postural changes; any manipulation of the infant; progressive desiccation of the Wharton jelly; or many other factors.
