We conducted a global study of HRCT interpretation among an international group of radiologists and pulmonologists. Participants evaluated HRCTs from 203 suspected IPF patients, assigning a likelihood score for each of the guideline-based HRCT categories (each 0-100%, summing to 100%), an ILD extent score on a 3-point scale (1-25%, 26-40%, 41-100%) and a probability of progressive and stable disease (each 0-100%, summing to 100%). Diagnostic agreement was measured using weighted kappa coefficient (κw). Survival was evaluated using Cox proportional hazards and subgroup comparisons made using the C-index. Observer agreement applying current guideline-based HRCT criteria is moderate. Baseline assessments of UIP-like features, disease extent and likelihood of progressive disease independently are associated with mortality in patients with suspected IPF

Abstract
