A complex anatomic variant involving the right lung and characterized by upper lobe agenesis and left vascular isomerism is extensively demonstrated through the correlation of bronchoscopic, computed-tomographic, and surgical findings. The knowledge of this developmental anomaly can be useful to reliably localize endobronchial lesions and correctly plan possible lung resection.
