The present study is based on a case report of anterior cruciate ligament (ACL) revision after reconstruction with patellar tendon (PT) and femoral fixation with transcondylar screw. Treatment of this case showed that transcondylar femoral fixation of PT graft can increase the risk of vertical graft placement, causing a recurrent rotational instability of the knee. Furthermore, we observed that, at the time of surgery, transcondylar screw must be removed as it can interfere with the new femoral tunnel. Finally, the hole left by transcondylar screw has to be filled with bone graft in order to avoid the formation of a fistula between the femoral tunnel and screw hole.
