Qualità e modelli di cura. Una verifica sulle riforme in sanità

This volume shows the outcomes of two surveys, one concerning the quality of the service noticed in 32 public and private hospitals across Milan and its province, and the other concerning how, in the urban context of Milan, the mental disease is faced by the services provided in the national and regional health programme. The first study focuses on the quality “perceived” by 1100 users of the service, who were submitted a face to face questionnaire. The attention is mostly directed to what is stated in the art. 14, D. Lgs. 502/92 i.e. the humanization and personalization of the health attendance, the right to information, the degree of participation and the quality of the board and lodging. The second has been accomplished through 52 in-depth interviews to subjects, working in the psychiatric field, in order to underline how the problem of the closure of the old madhouses is faced in Milan (obstacles, followed patterns, expectations of the workers and consequent proposals). The interpretation of the outcomes of both surveys is oriented in order to verify on one hand the effects of the health reforms carried out in Italy and, on the other hand, the consequences of the change occurring in the relationship between doctor and patient and, more in general, in the collective conception of health.

Questo volume riporta i risultati di due ricerche, una sulla qualità del servizio riscontrato in 32 ospedali pubblici e privati di Milano e della sua provincia e l'altra sulle modalità con le quali nel contesto urbano milanese si affronta il disagio psichico mediante i servizi previsti dal piano sanitario nazionale e regionale. Il primo studio analizza la qualità “percepita” da 1100 utenti, ai quali è stato somministrato un questionario face to face. L'attenzione è rivolta soprattutto a quanto prevede l'art. 14 del D. Lgs. 502/92 e cioè l'umanizzazione e personalizzazione dell'assistenza sanitaria, il diritto all'informazione, la partecipazione e la qualità delle prestazioni alberghiere. Il secondo è stato realizzato mediante 52 interviste in profondità a soggetti che lavorano nel campo della psichiatria allo scopo di evidenziare come si affronta nella metropoli milanese il problema della chiusura dei manicomi (ostacoli, modelli perseguiti, aspettative degli operatori e proposte conseguenti). La lettura dei risultati di entrambe le ricerche viene effettuata nell'ottica di verificare da un lato gli effetti delle riforme sanitarie realizzate in Italia e, dall'altro, le conseguenze del cambiamento oggi in atto nel rapporto medico-paziente e, più in generale, nel concetto di salute.
